Treatment Request Form
The clinic requires a minimum of 48 hours’ notice for any appointment cancellation or rescheduling.
Where an appointment is cancelled or rescheduled with less than 48 hours’ notice, the clinic reserves the right to apply one of the following:
- forfeit the treatment or treatment session in full
- charge a cancellation fee equal to 50% of the full treatment cost
